Community leaders aiming to improve health equity and tackle the social determinants of health across the M Health Fairview service area are invited to apply for the Center for Community Health Equity Accelerator Fellowship 2027. Selected participants in this 12-month program will receive professional mentorship, leadership development, and up to $10,000 in project funding to help transform their community-centered concepts into tangible solutions.
